# Choosing an allergy compounding pharmacy

The pharmacy that mixes your patients’ allergy vials shapes safety, potency and your staff’s week. These are the questions that separate a dependable allergy compounding partner from a generic one—and how AlleReach answers them.

For physicians, practice managers and purchasing teams choosing a pharmacy to compound patient-specific allergen immunotherapy. Use it alongside written terms from each pharmacy you evaluate. For a side-by-side look at specific pharmacies, see our pharmacy comparison.

## USP <797>: ask for the evidence, not the logo

USP <797> is the sterile-compounding standard, and since November 1, 2023 it includes a dedicated section for allergenic extract prescription sets—vials of premixed licensed extracts diluted for one patient’s subcutaneous immunotherapy. Section 21 sets the minimum: trained and supervised compounders who pass written testing, gloved-fingertip sampling and an annual media-fill test; an ISO Class 5 primary engineering control or a dedicated allergenic extract compounding area; daily cleaning; defined labels; shipping controls; and written records.

A pharmacy cleanroom can go beyond that minimum. Ask which standard applies to your prescription sets and ask to see the proof: standard operating procedures, personnel competency records, certification reports for the hood or cleanroom, refrigerator temperature logs and the compounding record for each set.

AlleReach operates a licensed 503A facility and clean room that meets USP <797> requirements, with an on-site Pharmacist in Charge, Dr. Kristi Kubosh, PharmD, RPh, MBA, and Dr. Erica Noelle Leger, PharmD, who brings more than 10 years of sterile and non-sterile compounding experience.

## Beyond-use dating and stability

Under USP <797>, a prescription set’s beyond-use date can be no later than the earliest expiration date of any extract or diluent in it, and never more than one year from the day it was mixed or diluted. The practice parameter notes that prepared allergenic products usually carry dates of 3 to 12 months, again capped by the shortest-dated component.

Ask how the pharmacy assigns each date and what supports it. Potency depends on concentration, diluent, protein content and temperature: more dilute vials lose potency faster, which is why diluents with added protein such as human serum albumin are used to improve stability.

A good pharmacy also tells you what to do when dates and real life collide. AlleReach asks practices that dilute a concentrate to do so promptly, and issues new vial and box labels with the correct expiration dates if dilution will take longer than three days.

## Extract sourcing

Allergy immunotherapy vials should be made from FDA-licensed manufacturer’s extracts. Ask which manufacturers the pharmacy uses for each allergen, whether the extracts are standardized (BAU or AU) where standardized products exist, and how lot numbers are tracked to each patient’s set.

Consistency matters more for allergen extracts than for ordinary drugs. The practice parameter warns that switching lots, manufacturers or formulations—routine for conventional medications—can change composition or potency enough to cause a reaction. Ask the pharmacy to notify you before any change of manufacturer for a patient on maintenance, so you can decide whether to adjust the dose.

## Diluents, preservatives and phenol

Every multi-dose allergy vial needs protection against microbial growth. The practice parameter’s extract-preparation standard requires dilutions to be bacteriostatic: at least 0.25% phenol, or at least 20% glycerin when phenol is lower. Phenol is the preservative most often used in sterile allergy diluents.

AlleReach discloses its diluents plainly. It uses two: normal saline with phenol (NSP), a buffered saline solution containing 0.4% phenol, and human serum albumin diluent (HSA), the same NSP solution with 0.03% human serum albumin added for allergen stability. Both contain 0.4% phenol as a bacteriostatic agent. AlleReach also explains the occasional fine particulate that can appear in these diluents and when to call the pharmacy about it.

Ask every pharmacy for the same detail: which diluent goes in each vial, its phenol and glycerin concentrations (with units) and how the final concentration changes as vials are diluted. Our ingredient guide explains how to read those numbers.

## Stability in transit and on arrival

USP <797> requires shipping methods expected to deliver prescription sets undamaged, sterile and stable, with handling instructions on the outside of the container when special handling is needed. Ask how the pharmacy packs for heat and cold, which carrier it uses and how it handles delays.

AlleReach ships every extract order overnight by courier, packed in line with studies of allergen extract stability at ambient temperatures, and adds summer ice packs on request. Vials do not need to arrive cold; after a prolonged delay or extreme temperatures, the pharmacy assesses potency before you use the set. Once received, injection vials are refrigerated.

## Turnaround and ordering

Ask for the clock in writing: when an order counts as received, how long compounding takes, when it ships and when it arrives. Ask how missing information on a prescription is resolved and how quickly.

AlleReach is built for speed from a central location in Dallas, with convenient shipping across the continental United States and just-in-time deliveries for orders needed within 24 hours or less. New and existing prescriptions can be started by phone, and established practices reorder through a refill portal.

## Prescription accuracy

Most compounding problems start with an ambiguous prescription. A dependable pharmacy reads every order back against the patient’s allergen list, flags incompatible combinations (for example, high-protease extracts that can degrade other allergens) and calls before it guesses.

Check the finished label. USP <797> requires each vial to show the patient’s name, the type and fractional dilution with a matching vial number, the beyond-use date and storage conditions. A consistent numbering or color system across the set reduces administration errors.

## Clinical support and billing

You will have questions after the order ships: a missed dose, a reaction, a vial that looks different. Ask who answers, when, and whether a pharmacist is available.

AlleReach Customer Support is available 8 am to 7 pm Central, Monday to Friday, and answers emails to RX.Support@AlleReach.com within 24 hours or by the end of the next business day. The pharmacy line is 972-913-2938. For billing, the team shares general best practices on payers and billing; customized claim guidance and appeal support are not part of the service, and AlleReach is building a list of independent billing vendors for practices that want that help.

Sources: [USP <797> Section 21: compounding allergenic extracts (AAAAI excerpt)](https://education.aaaai.org/sites/default/files/media/2023-05/Section%2021%20Allergen%20Extracts.pdf); [AAAAI/ACAAI: Allergen immunotherapy practice parameter, third update (2011)](https://www.aaaai.org/Aaaai/media/MediaLibrary/PDF%20Documents/Practice%20and%20Parameters/Allergen-immunotherapy-Jan-2011.pdf).

### Why do allergy vials contain phenol?

Phenol is a preservative that keeps multi-dose vials bacteriostatic. The practice parameter requires at least 0.25% phenol, or at least 20% glycerin when phenol is lower. AlleReach’s NSP and HSA diluents each contain 0.4% phenol.

Sources: [AAAAI/ACAAI: Allergen immunotherapy practice parameter, third update (2011)](https://www.aaaai.org/Aaaai/media/MediaLibrary/PDF%20Documents/Practice%20and%20Parameters/Allergen-immunotherapy-Jan-2011.pdf); [AlleReach FAQ: Diluents and visible sediment](https://www.allereach.com/faqs/why-does-it-appear-that-there-is-visible-sediment-in-the-master-vial).

### What is HSA diluent?

Human serum albumin diluent adds a small amount of protein (0.03% in AlleReach’s HSA) to phenolated saline. The added protein improves allergen stability, especially in more dilute vials.

## Regulatory context

Allergy shot prescription sets are prepared for one patient from FDA-licensed allergenic extracts and an appropriate diluent, under the USP <797> standards for allergenic extracts. Personalized allergy drops are compounded and prescribed off-label, made from the same FDA-approved allergenic extracts used in allergy shots; off-label does not mean unsafe or experimental.
